Christmas Pageant Permission Slip 2009

has my permission to be in The Unitarian Church in Westport Christmas Pageant on Thursday, December 24 at 4pm.

My child would like to be:

Friday, December 18 at 4:30 pm
Thursday, December 24 at 2pm

I must pick up his/her costume after the rehearsal on December 18.
